I am saddened, but I have to drop my rating. Extension no longer blocks video on Twitter. Should developer fix this issue, I'll once again give it 5 stars.

Click the extensions button (puzzle piece by the setting button) and click this extension to turn the extension on. It doesn't do anything by default. Once on it sort of works. Images are replaced by simple black boxes. They still take up space, but it's way less distracting.
